A family home provider or adult foster care provider in a family oriented setting. Supports provided include training in activities such as laundry, routine household care, self care, shopping, money management, socialization and leisure activities.
This service supports participation in daily meaningful activities and routines in the community. It includes participation in regularly scheduled activities in a non- residential settings that are designed to support the development of skills, build positive social behavior, support friendships, foster greater independence and personal choice. It is also designed to include career planning, pre-vocational activities, supports to maintain skills and functioning and to prevent or slow regression as well as supported retirement activities.
This service is available only to those living in their own residence or with their family and who receive the SCL waiver. This support is provided one on one and is designed to provide assistance and foster independence with home and community based activities.
One on one training or assistance for a person who lives at home with their family or in their own home and receives the MP waiver. This service is designed to provide assistance with community based activities as well as promote independence with at home activities.
This support offers rest or relief for the primary care givers of a person receiving services and supports. This service is available for those living at home with their family.
